NOTE:  While we are here to help, knowing your insurance benefits is your responsibility.  Please contact your insurance company with any questions you have regarding your coverage.  Valid insurance cards must be presented at the time of service.  Please be aware that the balance of your claim is your responsibility whether your insurance company pays for your claim or not.  Your insurance benefit is a contract between you and your insurance company.

Family Practice Associates is happy to offer an easy and secure way to pay your bill online, without the need for a username or password.
